B.Sc Honours in Medical Laboratory Sciences graduates
51st Convocation of the University of Sri Jayewardenepura will be held on 15th, 16th and 17th October 2025 at the Bandaranaike Memorial International Conference Hall for the conferment of degrees. The session for Faculty of Allied Health Sciences will be held on Friday, 17th October 2025.
Eligible graduates of the Faculty of Allied Health Sciences are required to submit the completed convocation supplication forms.

First Degree Rs. 6,500.00
First Degree (in absentia) Rs. 6,500.00
Dully filled supplication forms along with the scanned copies of the following documents (must be clear scanned documents in PDF format (not images, photos taken from cameras, mobile phones or shaded/darked documents scanned by mobile phones.) should be sent on or before 20.08.2025 via con.fahs@sjp.ac.lk
- Dully filled supplication form and the declaration (A4 size)
- The bank receipt for the payment of the convocation fee (yellow copy for the bank payment/ generated payment slip for online payment)
Subject of the e-mail should be as follows:
Index number, Registration no, Name of the Degree
Ex : HS/NSG/xxxx, HSxxxx, B.Sc. Nursing Degree
You should make sure to send the above documents via your personal e-mail. If you have any inquiry, please contact Faculty’s office, Faculty of Allied Health Sciences (contact numbers: 0112881632 Extension 1632)
All originals of the above e-mailed documents together with a self-addressed (23 cm x 10 cm) Rs. 60 /= stamped envelope stating the index number, registration number and the name of the degree on top left-hand corner (as mentioned in no. 03 of the Convocation supplication form) should be sent by registered post to the following address to reach on or before 22.08.2025 or handover directly to the Faculty office, Faculty of Allied Sciences.

Extra information
In the original degree certificate, your name in Sinhala will be appeared as on your birth certificate, and in English it will be appeared as in the final results sheet. The changes done by you in the name in Sinhala or English in the Supplication Form will not be accepted and do not change your name.
Cloaks will be issued by the Faculty of Allied Health Sciences on a week prior to the Convocation. You should handover your Student Identity Card to the Faculty Office on that date and ensure to return your Library Books before that date.
